Abstract

Kolorektale Karzinome entwickeln sich typischerweise über eine Adenom-Karzinom-Sequenz. Die lange Latenz der Karzinomentwicklung ermöglicht die gezielte Einflussnahme auf bekannte Risikofaktoren und die Früherkennung der Erkrankung in einem kurativ behandelbaren Stadium. Die Veränderung «typischer westlicher» Lebensgewohnheiten stellt eine erwiesene Maßnahme zur Prävention des kolorektalen Karzinoms dar. Nach gegenwärtiger Datenlage sind regelmäßige körperliche Bewegung, eine vitamin- und ballaststoffreiche Kost, Nikotinkarenz und nur mäßiger Alkoholkonsum am besten geeignet, das Auftreten kolorektaler Karzinome zu verhindern. Die Datenlage zur Chemoprävention stellt sich weniger überzeugend dar, eine generelle Empfehlung zur Chemoprävention beispielsweise mit nichtsteroidalen Antirheumatika (NSARs) oder den viel beworbenen Mikronutrients kann nicht ausgesprochen werden. Jede dieser Präventivmaßnahmen kann nur eine Ergänzung, nicht aber einen Ersatz zur Vorsorgekoloskopie als Goldstandard der Prävention kolorektaler Karzinome darstellen. In der Ausbreitungsdiagnostik des Rektumkarzinoms stellt die Endoskopie mit flexibler und starrer Rektoskopie und dem endorektalen Ultraschall zusammen mit der Oberbauchsonographie und der CT die entscheidenden Pfeiler der Diagnostik dar. Die Indikation für eine CT-Kolonographie und/oder FDG-PET ergibt sich aktuell nur in Einzelfällen oder im Rahmen von Studien.
